NCCS adds care planning to Teamwork resource
NCCS has revised and updated Teamwork: the Cancer Patient’s Guide to Talking with Your Doctor booklet to include a sample treatment plan with a map to appropriate questions for each component to ensure the patient remains an active team member and decision maker in their care. A treatment summary template and sample survivorship care plan have also been included to put care planning into context and demonstrate what should be expected during and after treatment. Many of the changes are a direct result of input from a variety of stakeholders such as survivors, nurses, physicians, and social workers.
Teamwork is a resource to help those diagnosed with cancer and their caregivers find better ways to communicate with their health care team. The booklet focuses on improving the communication and decision-making process between patients and their health care team through providing lists of appropriate topics and questions to ask during various points in the cancer experience. Some topics addressed include:
- Diagnosis
- Managing symptoms and side effects
- Sexuality
- Clinical trials
- End of life
