Activity Submission Form

If you are an NCCS-connected advocate and actively engaged in survivorship advocacy, like policy advocacy, research advocacy, or community advocacy, please use this form to let us know about your advocacy activity and achievements. Your advocacy work could be highlighted across our social media in the NCCS Advocacy in Action campaign. Help us celebrate your dedication and amplify your advocacy work across the nation! Together, let’s inspire and drive positive change toward quality cancer care.

Please note: Advocacy activities should have taken place within the last 30 days.

Priority will be given to advocacy work such as:

  • Hosting a survivorship event (non-fundraising).
  • Having an article or blog published in a regional or national publication.
  • Making a media appearance (news media, TV, radio, podcast).
  • An academic presentation (panel participation, presenting a poster or research).
  • Receiving an award.
  • Being selected to serve on advisory committee or similar.
  • Legislative advocacy (meeting with lawmakers).
